Check-In Health Screening Questionnaire

As part of the check-in flow, parents & staff will be presented with a health questionnaire to fill out and track the health of your center.

The brightwheel team understands the importance of maintaining a safe and healthy environment for students and staff members. The Health Screen option at check-in was designed to help schools both meet regulations implemented due to COVID-19 and help to provide concrete measures to maintain a healthy space for the communities you serve. The Check-in Health Screening can be used in conjunction with brightwheel's Check-in Kiosk and Quick Scan features to minimize the touching of communal surfaces.


Enable Health Screen

Administrators can enable the Health Screen feature from the Check-in Settings on the brightwheel mobile app or on the web. By enabling this option, parents and staff will be required to fill out a health questionnaire at check-in.

On the Web

  1. Click on your name in the upper right-hand corner and click School Settings

  2. Choose the Check-in tab

  3. Scroll to the bottom of the page and toggle the Health Screen option on for students and/or staff

On Mobile

  1. From the Administrator Home screen, tap School Profile

  2. Under School Settings, tap Check-in Settings

  3. Toggle on the Health Screen option (this works with any other setting you have enabled)

  4. Tap Save


Customize Health Screen

Many states and jurisdictions have unique requirements with regard to health screens and COVID-19. Brighthwheel’s Health Screen feature allows administrators to customize statements to be confirmed by parents. Modification can be done from the School Settings page online.

  1. Click on your name in the upper right-hand corner and click School Settings

  2. Choose the Check-in tab

  3. Scroll to the bottom of the page and click the Customize button under the Health Screen option

  4. Modify any current statement or add a new one by clicking + Add Another Statement

  5. Click Save to complete

Up to 14 statements can be added to the screening questionnaire!


Checking in with Health Screen

Once the Health Screen option is enabled, a new screen will appear while the parents and/or staff member completes the check-in. This screening asks the person completing the check-in to confirm all statements by checkboxes. For example, the default questions are:

  • No fever

  • No cough or shortness of breath

  • No contact with COVID-19 in the last 14 days

If the parent or staff member failed to check all of the boxes, they will be presented with the following message: 'Please discuss your health screen with your provider to confirm it's okay to check-in.'


Failed Health Screen Notification

When a parent fails to check all the questions on the health screen questionnaire, by mistake or intentionally, they receive the above pop-up. If they choose to continue with the check-in, administrators will receive a 'Health Screen Alert' push notification alerting them that the student failed the questionnaire. Tapping on the notification will immediately open that student's feed to display the most recent check-in and which questions were left unchecked.


Health Screen in Student Feed

When students are checked in with the Health Screen, their check-in record will display on the student's feed as it always has. The answers to the screening questions will appear above any note added to the record.


Health Screen Reporting

The result of the student's Health Screen Questionnaire can be reported on from the Check-in Report. By checking the Show Health Screen Results box, a new column will be added to show whether a student passed the questionnaire or failed. You can see which question was vailed by clicking View. This is also exportable.

The results of staff health screens can be located in the mobile app by tapping Staff from the Administrator Homes screen. Tap on View Timecards at the bottom to view staff health screens. This information will also be included on the staff member's Timecards report.
