[For Guardians] Learn how student contacts can complete the Health Screen questionnaire during check-in or at home

If enabled by the program, health screening questionnaires can be completed during student check-in, or at home directly from the brightwheel mobile app! Student contacts listed as Parent, Family, or Approved Pickup can complete a health screening questionnaire during student check-in.

Only Parent contacts can submit a home health screening questionnaire, allowing administrators to review and accept this information prior to the student check-in.

💡 Not sure if your child’s program requires Health Screens? Please reach out to a program administrator and inquire how they’d like these to be completed.


Complete a Health Screen at Check-in

If enabled by the program, Parent, Family, or Approved Pickup contact types will be required to complete a Health Screen Questionnaire during the check-in flow. A new screen will appear requesting confirmation statements by checkboxes.

Please visit our Check-in Students as a Guardian resource for steps on checking a student in and completing a health screen.

If a student contact fails to check all of the boxes, they will be presented with the following message: 'Please discuss your health screen with your provider to confirm it's okay to check in.'


Complete a Health Screen at Home

If enabled by the program, Parent-level contacts can submit a Health Screen Questionnaire outside of the check-in flow on the brightwheel mobile app. The completed health screen will appear on the student's activity feed.

  1. Log in to brightwheel on the app

  2. Tap the student profile that the health screen needs to be completed for

  3. Tap the Add Activities then tap the Health Screen activity icon
    Please note: If Add Activities is unavailable, tap Health Screen

  4. Fill out the health screen questionnaire and press Submit

  2. Tap the student profile that the health screen needs to be completed for

  3. Tap the purple arrow icon on the bottom-right of the page

  4. Select the Health Screen activity option

  5. Fill out the health screen questionnaire and press Submit


Manage Home Health Screens

Once a home health screen activity has been posted to a student’s feed, this activity can be edited or deleted by the parent-level contact or an administrator of the program.

ℹ️ Health Screens logged during student check-in cannot be edited by parent-level contacts and must be edited by a program administrator.

Delete Home Health Screens

  1. Log into brightwheel on the web

  2. Click My Children

  3. Click to open the applicable child's profile

  4. Jump to the Feed tab

  5. Locate the parent post Health Screen activity and click Edit

  6. Click 🗑️ Delete Activity

  1. Log in to brightwheel on the app

  2. Tap on the student’s profile

  3. Scroll on the activity feed to find the parent Health Screen

  4. Tap directly on the health screen activity

  5. Tap Delete

Edit Home Health Screens

  1. Log into brightwheel on the web

  2. Click My Children

  3. Click to open the applicable child's profile

  4. Jump to the Feed tab

  5. Locate the parent post Health Screen activity and click Edit

  6. Edit the date/time/notes as needed

ℹ️ At this time, home Health Screens can only be edited from the web version of brightwheel.
