Providers can easily collect multiple digital signatures from guardians on a single form. Simply add multiple signature sections to the form, and this allows each guardian to sign the form, either from their own account or when completing the form together.
ℹ️ Guardian instructions: After the first guardian signs the form, they should save it. This allows the second guardian to access the form, add their signature, and then submit the form.
Add multiple signature fields
Once you create a new form, click + Add section > Parent signature
2. Once the Digital signature section is added, hit Create new field within that section to add a second signature field.
3. Under 'Field type', choose Drawn or Typed signature. Label the field (E.g. Parent signature and add the field. This can be repeated for as many signatures as you like.
💡 Tip: If you're unsure how many guardians will sign a form, we recommend only required one signature and setting the others as optional. This provides flexibility while ensuring you get at least one necessary signature.
